Requirements:

1. Personal appearance of applicant and two witnesses.
2. Two passport size photographs taken within three months.
3. Provision of original valid Pakistani passport or CNIC card.
4. Unsigned original power of attorney.
5. Applicant and witnesses will need to sign the power of attorney in the presence of Embassy’s
   official.
